Trev first appeared in CARS and Mattel got as far as the prototype and production ready stage but never pulled the trigger for an actual release ... it probably was in line with the size of a hauler?

The final prototype and production ready look.

Mattel creates a prototype based on the screenshot supplied by Pixar. A 3D prototype is produced. If I had to guess, I would say the front cab area would be metal (you can see the lines) but the main body and his expression plate would be plastic (I removed the color in the areas where I presume the final one would be plastic but again, just my guess).

Once approved, it gets scheduled and produced in quantity.
While we did not get a Trev release in the mainline, we did get tis cute carrying case for Mini Racers.

This would have sold like hotcakes. What an iconic scene -- one that most kids would happily reenact with their diecasts, if Mattel ever releases it!
Based on his axles and nose, Trev is a later model of what is called an "E Unit" (made by the Electro-Motive Division -- EMD -- of General Motors). An E unit would be 10.5 feet wide, 15 feet tall, and 71 feet long -- so in theory, perhaps a bit bigger overall than a Hauler.
